Admissions Forms


All Admissions forms should include your full name, date of birth and University ID number (once assigned). Unless otherwise specified, you may either fax completed forms to (813) 974-9689, email to international@usf.edu, or mail to:

Immigration & Financial Forms
The International Student Services Office is converting all paper request forms to online electronic forms (E-Forms). These E-Forms will be made available in USF iStart, a comprehensive data system for case management and immigration compliance. Students can access the E-Forms at their convenience using most electronic devises and web browsers.

To use iStart and access the E-Forms, students should go to https://istart.usf.edu and enter their USF NetID and Password to login. Using the menu on the left, students can pick the E-Form they need and then follow the instructions for completing it. Processing time and additional requirements are listed on each E-Form. When the request is ready, the student will be notified by email, and if applicable, may visit the OIS office to pick up their requested documents.

Current Student Forms

  • Transcript Request Form - This form can be used to request an official transcript (at a cost of US$10 per copy) with the Office of the Registrar.
  • FERPA Waiver Request Form - This form is provided as a means for students to give the University of South Florida permission to discuss and/or disclose their academic records with someone other than themselves (i.e., with a parent, guardian, etc.
