Request an OASIS
ID
A personal OASIS ID
is required for accessing the USF Student Information System as well
as the USF Foundation Alumni and Finance Systems. Please
follow the steps below to request your OASIS ID.
Job Change or New Job
Assignment:
If you have moved to
another university department due to a job change or have been
assigned new job responsibilities please contact Suzanne McCoskey,
Registrar's Office at
smccoske@admin.usf.edu. to request
an Access Re-Assignment form.
Step 1 - FERPA Requirement
Please Note: To
request an OASIS ID all users will be required to complete
the Family Educational Rights & Privacy Act (FERPA)
tutorial. Records in the USF Foundation's Alumni
database are not subject to FERPA; however, may users of Alumni
records also access student records in OASIS. Therefore, all users must
complete the FERPA tutorial prior to obtaining an OASIS ID.
This practice ensures the University is in compliance with
governmental requirements regarding the release of student
information.

Step 2 - Request A
Personal OASIS ID

Complete the Request for OASIS (BANNER) Access
form. This form is only available after the FERPA
Tutorial has been completed.
This is the official
form used by USF.

on the form.
