Dean Certifications are forms completed by the Office of Student Rights and Responsibilities certifying whether a student has a disciplinary record at USF. These forms may be completed for the following reasons:
1. Transfer applications,
2. Graduate school applications,
3. Medical school applications,
4. Any other applications that require this information (including employers).
Forms must be completed by the student/former student and submitted to the Office of Student Rights and Responsibilities at the following address:

If the school/employer that will be receiving these forms accepts faxed copies, the form may be faxed to the Office of Student Rights & Responsibilities at 813-974-7383.
Forms may also be sent via email to: firstname.lastname@example.org
The student must indicate the manner in which they would like the form returned to the school/employer (e.g. mailed, faxed or emailed).
If mailed, the student must provide an address if it is not clearly indicated on the form.
