Voluntary Submission of COVID-19 Vaccination Records

Although USF is not requiring proof of COVID-19 vaccination, the university strongly encourages everyone to be vaccinated. Proof of your COVID-19 vaccination can be voluntarily submitted through this link. 

If you require a 2nd vaccination, the State of Florida has made vaccines widely available to everyone medically eligible. For the convenience of our students, faculty and staff, our campuses are hosting weekly vaccine clinics. For more information on Vaccine clinics, click here
