Responsible Employee Reporting

If you become aware of an incident which you believe may constitute sexual harassment, sexual violence, or gender discrimination, please complete the Title IX Incident Report Form immediately. If you have any questions on how to complete the form, please contact the DIEO Office at 813.974.4373 for assistance.

Upon receipt of the Incident Report Form, the Title IX Office will contact the appropriate individuals for further information.

The Incident Report Form (for Non-Title IX) is to be completed by any person who would like to notify DIEO that an incident has occurred that is covered under the above referenced policies.

Anonymous Reporting

This is not a reporting option for mandatory/responsible employees.

USF has engaged EthicsPoint—a third party hosted hotline—to enable the safe, secure, and anonymous reporting of activities which may involve misconduct, fraud, abuse, and other violations of USF policies. USF's Retaliation, Retribution, or Reprisals Prohibited Policy 0-020, prohibits retaliatory action be taken against anyone who reports or inquires in good faith about potential breaches of USF policies or seeks guidance on how to handle suspected breaches.

Reports made in EthicsPoint and associated investigatory records will be kept confidential to the extent possible under applicable state and federal law. Be advised that revealing your identity in EthicsPoint may result in disclosure of your identity in response to a public records request under Florida's public records law [Chapter 119, Florida Statutes].

DO NOT use EthicsPoint to report the following:

If you are a Responsible Employee fulfilling reporting obligations under Title IX. You MUST submit a Title IX incident form.

Academic Matters involving faculty and/or students should be reported directly to the Office of the Provost and Executive Vice President.

Non-Academic Student Conduct Matters should be reported directly to the Office of Student Rights and Responsibilities or by calling 813-974-9443.


EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

General Reporting (Non-Title IX related)

Students, staff, and faculty are strongly encouraged to report allegations of discrimination, harassment, and/or retaliation covered under the University's Diversity and Equal Opportunity: Discrimination and Harassment Policy #0-007; Sexual Misconduct/Sexual Harassment (Including Battery) Policy #0-004; and/or Disability and Accommodations (Public/Employees/Students) Policy #0-108. Delaying the immediate reporting of an allegation often hinders a proper investigation and can also unnecessarily subject the Complainant to continued discrimination, harassment, and/or retaliation.
