File an Incident Report
Title IX protects people from sex discrimination in educational programs and activities at institutions that receive federal financial assistance. 20 U.S.C. § 1681. Title IX applies to USF programs and activities, as related both to education and employment. USF is committed to providing an environment free from sex discrimination, including sexual harassment and sexual violence. If you wish to file a complaint because you feel you have been sexually harassed, the target of sexual violence, or discriminated against because of your gender, please contact the Title IX Office for more information or complete the form by clicking the button below.

If you are unsure how to file a complaint or wish to speak with someone regarding your concerns you may contact Title IX at (813) 974-8616.
A complaint may be mailed, emailed, or hand delivered to Title IX Office. A Complaint must include a detailed summary of what you are alleging happened and how you wish to have the complaint resolved. A member from the Title IX team or any Title IX Deputy Coordator is available to meet with you to discuss all your options available to resoving a sexual harrassment complaint and the process for each option.
1. Complete the Title IX Incident Report Form.
2. For Title IX complaints, email the form to: titleixreports@usf.edu or mail/deliver form to:
