Laboratory/Studio & Field Incident Report
The Laboratory/Studio and Field Incident Report form is to be completed by the Lab Manager/Teaching Assistant/Instructor for any incident that occurs in any University of South Florida affiliated teaching or research laboratory/studio or field research project.
If an accident or incident resulted in a work-related injury or illness, your supervisor must be contacted immediately. Please proceed to the Workers' Compensation website, for more information on how to report a work-related injury or illness.
Lab Safety Around the Country
In October 2011, the U.S. Chemical Safety and Hazard Investigation Board (CSB) issued a report on their investigation of a chemistry lab explosion that occurred at Texas Tech University. The report also highlighted key findings from other serious accidents that occurred at various universities.
