You can access a certified copy of your immunization records for yourself or to use with other universities. 1. Log into your OASIS account 2. Select “Personal Information” 3. Choose “View Immunization Record” 4. Print your immunizations

Use the link below to access your OASIS account for a certified copy of your immunization record. If you find that additional records are needed, please complete a Medical Records Request with USF Health. To contact our office, please click the link far below.
Returning Students
To see if your previously submitted records meet the revised immunization policy, log into your OASIS account, select Personal Information, and select View Immunization Record. Recommended vaccinations include the Hepatitis B and Meningitis vaccinations. Tuberculosis Screening is required for students that applied to USF using an international address.
