Commendation or Complaint
We are a professional state law enforcement agency and are here to serve the campus community as well as protect it. USFPD officers and employees are held to the highest standards of their profession and we welcome any compliment or concerns regarding our performance. Commendations and complaints may be made in person, by telephone, mail, or email. This completed form can be emailed to email@example.com.
Crime Prevention and Awareness Programs
The crime prevention and awareness programs offered to the community by the University of South Florida Police Department are listed within the request form. Please complete the form in its entirety. A member of the crime prevention section will contact you within 3-5 business days to discuss your request.
Event Request Form
Events on campus should be submitted via the USF Event Request Form. USFPD will contact you if we need more information about your event.
USFPD is responsible for fulfilling public records requests for copies of law enforcement reports in accordance with the Florida Public Records Law.

Request for Therapy K-9 Bailey
Please complete the information in this survey to request Bailey and her handler to appear at an event. Please allow at least seven (7) days for the request to be received, reviewed and scheduled. If you have a shorter time frame, please complete the survey and then contact our Communications Center at 813-974-2628 and ask to speak with the K9 Therapy Handler.
