USF students who are currently registered for classes at the Tampa campus and St. Petersburg campus are eligible to receive services at SHS. Students registered at the Sarasota campus are eligible if they voluntarily pay the Health Fee (please call 813-974-2331). You will need to present your current, valid USF student ID card when you come into the clinic. If you are taking a semester off (e.g. enrolled for Fall 2017 but not taking classes Summer 2017), you can access services at SHS during the off-semester if you pay a Health Fee. USF graduates who can show proof they will be continuing in a USF program within 2 semesters following graduation may also pay a health fee to access services between enrollment.

No Show Fee
If you have an appointment that you cannot keep, please call SHS at least 2 hours before your appointment time to cancel or reschedule. If you fail to cancel and do not show up for your scheduled time, you will be charged a $10 No Show fee.
Late / Tardy
When you are late, it causes other scheduled appointments to be delayed, which isn't fair to other patients. Therefore, if you arrive more than 10 minutes late for your scheduled appointment, you will be rescheduled. We recommend that you aim to arrive 10 to 15 minutes ahead of your scheduled appointment to ensure you have plenty of time to find parking, check in, complete any necessary paperwork and relax for a moment before seeing your provider.
Students may experience medical conditions, psychological conditions, illnesses or injuries that require them to be away from the classroom or workplace, or that affect their ability to perform optimally in class or on exams.
Per USF policy, all students must comply with the provisions of the University catalog. Attendance is the responsibility of the student and instructors have the discretion to approve or disapprove any absence. Although SHS may verify that you received care and offer a statement in support of consideration of an excused absence, only the instructor may excuse an absence. Unless you are a patient or a friend or family member escorting a patient, you will need to sign in to our Visitor Log at the front desk before entering any clinical areas.
The University of South Florida requires the following immunizations of all students:
MMR: All students born on or after January 1, 1957 must show proof of TWO vaccinations for Measles and Rubella
MENINGITIS: All students must show proof of vaccination or sign a declination.
HEPATITIS B: All students must show proof of vaccination or sign a declination.
Turn in your proof of immunization and the signed USF Medical History form to Student Health Services. Not complying will result in a registration hold. Visit the Immunization Compliance page for more details.
