Department Undergraduate Forms

Activities pertaining to a student's education need to be documented. The forms used by the Department of Computer Science and Engineering are listed below. These forms can be downloaded and printed out. Forms are to be submitted to the Department office.

Petition Form: General - The Department academic petition is for Department requirements only. An academic petition for late course add, drop, or change requires the university academic petition form which is administered by Engineering Advising.

Contract Form for Incomplete Grade - used to establish a contract between the student and the supervising professor for completion of an Incomplete ("I") grade.

Graduation Checklist for All Majors - click here to access more information regarding graduation and your graduation checklist. This needs to be completed the semester before you graduate and for permits to key graduation-required courses. 

FORMS FOR THE COMPUTER SCIENCE AND COMPUTER ENGINEERING majors

Contract Form for Independent Study (CIS 4900) - used to establish a contract between the student and the supervising professor for an activity outside of a classroom course for Computer Science and Computer Engineering. The Independent Study (IS) will not be substituted for a required class (it can be used for an elective credit). The IS provides an opportunity for the student to complete a project in an area not typically covered by a department course. The student must have a contract signed by the supervising professor and approved by the Department Chair or designated person to register for Independent Study.

Contract Form for Supervised Research (CIS 4915) – used to establish a contract between the student and the supervising professor for a supervised research experience for elective credit for Computer Science and Computer Engineering students. The student must have a contract signed by the supervising professor and approved by the Department Chair or designated person to register for Supervised Research.

Contract Form for Industry Internship (CIS 4940) – used to establish a contract between the student and the supervising professor for an industry internship for elective credit for Computer Science and Computer Engineering students. The student must have a contract signed by the supervising professor and approved by the Department Chair or designated person to register for Industry Internship.

Undergraduate Academic Regulations Committee Petition - used for readmission, late or retroactive drop, total withdrawal from entire term, late registration, late add, even exchange, and other petitions. For late adds, late or retroactive drops, and total withdrawals, a completed Instructor's Documentation Form is required.
