How to File an Equal Opportunity Complaint Form
USF DIEO Complaint Process & Forms
Students, staff, and faculty are strongly encouraged to report allegations of discrimination, harassment, and/or retaliation covered under the University's Diversity and Equal Opportunity: Discrimination and Harassment Policy #0-007; and/or Disability and Accommodations (Public/Employees/Students) Policy #0-108. Delaying the immediate reporting of an allegation often hinders a proper investigation and can also unnecessarily subject the Complainant to continued discrimination, harassment, and/or retaliation.

Equal Opportunity Incident Report Form
This form is to be completed by any person who would like to notify DIEO that an incident has occurred that is covered under the above referenced policies. Do not use this form if you wish to file a complaint. Please file a complaint using the Equal Opportunity Complaint Form that is in the next section below.
Upon DIEO's receipt of the Equal Opportunity Incident Report Form, DIEO will contact the appropriate individuals for further information. DIEO will evaluate the information provided and will determine if further processing is needed.
The Equal Opportunity Incident Report Form may be mailed, scanned and emailed, or hand delivered to DIEO.
Equal Opportunity Complaint Form
If you wish to file a complaint with the DIEO office, please use the form below. The Equal Opportunity Complaint Form must be filed with DIEO within 120 calendar days from the date of the alleged incident of discrimination, harassment, and/or retaliation.
