Title IX protects people from sex discrimination in educational programs and activities at institutions that receive federal financial assistance. 20 U.S.C. § 1681. Title IX applies to USF programs and activities, as related both to education and employment. USF is committed to providing an environment free from sex discrimination, including sexual harassment and sexual violence (See USF's Policy 0-004 Sexual Misconduct/Sex Harassment).
If you become aware of an incident which you believe may constitute sexual harassment, sexual violence, or gender discrimination, please complete the Title IX Incident Report Form below and return it to the DIEO Office at ALN 172 immediately. This form is to be completed by any person who would like to notify DIEO that an incident has occurred that is covered under the above referenced policies. Do not use this form if you wish to file a complaint. Please file a complaint using the Equal Opportunity Complaint Form that is in the next section below.
Upon DIEO's receipt of the Equal Opportunity Incident Report Form, DIEO will contact the appropriate individuals for further information. DIEO will evaluate the information provided and will determine if further processing is needed.
