• Supervisory Committee Form
  • Must be submitted prior to or at the same time as your Admission to Candidacy form
  • Any Committee Members who are not employed by USF must have a CV attached to the committee form
    
    
• Change of Committee Form
  • For making a change in the members of your committee or their position on your committee
    
    
• Deferment
  • In order to defer admission, *Admission can be deferred up to 3 semesters - after that you must reapply.
    
    
• Course Transfer Form
  • This is for internal and external course transfers.
    
    
• ETD Certificate of Approval
  • To be completed after successful defense
  • Our department does not sign this form.
    
    
• Permit Request Form
  • Submit all permit requests here, including Thesis and Dissertation Hour requests.
    
    
• MSME Plan of Study (Non-thesis)
  
  
• MSME Plan of Study (Thesis)
  
  
• Graduate Plan of Study (Ph.D.)
  
  
• Graduate Studies Petition
  
  • To add/drop or change hours for course registration, after the first week.
    
    
• Letter Request Form  
• Masters Defense Announcement Template
  
  • Must be in a JPEG format.

• Dissertation Defense Announcement
  
  • Must be in a JPEG format
  • Must be sent to Email.
    
    
• Independent Study Contract 
  • To be submitted with permits for Independent Study/Directed Research.
    
    
• Thesis / Dissertation Hours Contract
  
  • To be submitted with permits for Thesis/Dissertation Hours.
     
• Successful Defense Form
  • To be submitted after every Master's/Ph.D. defense
  • Please submit directly to Catherine Burton. 

• Successful Proposal Defense Form

  • To be submitted once proposal defense for qualifying examination has been completed.
    
  • Please submit to the Graduate Program Specialist.

• Defense Assessment Rubric
  • Bring a copy for each committee member to your defense.
  • To be submitted to the ME office only.
    
    

• Graduation Checklist

  • To be submitted to the ME office for advisor approval the semester before you graduate.
