Forms for Contracting

These are some of the forms you must bring for an Army ROTC scholarship and contract with the USF Suncoast Battalion Army ROTC program. 

Please be aware that the Recruiting Operations Officer will have you complete more forms in person during the contracting process.

104-R Form	Planned Academic Worksheet	This ensures Cadets stay academically aligned so they graduate on time.
Birth Certificate	Original Copy - will be returned
Social Security Card	Original Copy - will be returned
Standardized Test Scores	Documentation from SAT or ACT
Transcript	Current college transcript	Must have all completed credits and the previous semester's GPA
DD Form 2058	State of Legal Residence
SF 1199	Direct Deposit Signup Form	Use this version for USAA
