APPLICATION-USF “Non Degree Seeking” Application (Digital Online Application to USF for Admission to Dual Enrollment Program)

FORM 1-Dual Enrollment Informational Approval Form 1: Completed by students/parents (Digital-One time Every Year)

FORM 2-Dual Enrollment Approval Form 2: Course Planning (Digital Submission- For Each Semester one for Fall and one for Spring)
