DUAL ENROLLMENT FORMS

Student Application Link: The application link is found on the admissions page, non-degree, dual enrollment, then application.  CLICK HERE to begin.

FORM 1-USF DISCONTINUED FORM 1 as of SEPTEMBER 2025

Counselor Approval Form (CAF) submitted by the Counselor (Digital Submission- For Each Semester one for Fall and one for Spring per Florida Statute

DUE DATES: One CAF for FALL Mid-February to June 15th and then submit one for SPRING Mid-October to November 15th. The CAF needs to be submitted for each semester during submission dates only.

If an Academic Review Committee Petition (ARC) is being submitted, you first must notify the EUP office at dualenrollment@usf.edu to ensure the petition is properly routed. 

ARC Forms (2): All Dual Enrollment Students must complete and process both forms below:

ARC Instructor Form (Print, fill out form, add any extra documents and send to instructor for completion, then submit the form through Archivum in the self-service/student portal with the form below)

ARC Student Petition Form for Course Withdrawal  (Print, fill out form, add any extra documents to support your circumstances. Submit form through Archivum in the self-service/student portal)

IMPORTANT: If you move from private school or public high school to homeschooling or PEP, you must provide nondegree@Usf.edu and dualenrollment@usf.edu with a valid FL Homeschool or PEP acceptance letter (letter from the district or State) attached to the email.
