Academic Advising Survey
What is it? The Academic Advising Survey is a survey for you to complete after any interaction with your advisor regarding
your experience with the advisor.
When do I use it? After any interaction with your advisor.
College of Education: Change of Major
What is it? The Change of Major is a form to change your major from or within the College of
Education to another major within the College of Education.
When do I use it? You use after admitted to the College and if changing your major.
College of Education Petition
What is it? The College of Education Petition form is a form to request an exemption/modification from the academic policies within
the College of Education.
When do I use it? You should speak with your advisor about the policy and whether the petition is warranted.
What is it? Students are allowed to take a maximum number of 18 credit hours per semester in the
fall and spring and 14 credit hours in the summer. If you wish to go over that amount
of credit hours, you will need to fill out and turn-in a Course Overload Form to Student Academic Services.
When do I use it? After registration begins and before you register for more than the maximum number of hours allowed per semester.
What is it? This Course Substitution form allows students to request an evaluation of a transferred
course to determine if it is equivalent to a USF course(s).
When do I use it? When you transfer a course to USF and want to use it to fulfill a degree requirement.eSchedule an Advising Appointment with an academic advisor in Student Academic Services to receive the form and initiate the process.
What is it? This Time Limitation form allows former students returning and transfer students to request an evaluation
of USF courses and other institution courses taken over five years but less than ten
years ago to see if it is an equivalent to a current USF course(s).
When do I use it? It is recommended that students initiate the process while making a decision to enter the University of South Florida or prior to or during admission to the College of Education.
Declaration of Major
What is it? This form allows students to declare a major in the College of Education as a Pre-Admitted
student. Please be aware that the Declaration of Major is not the same as a formal
application to the College of Education.
When do I use it? If you are interested in declaring a pre-admitted major within the College of Education, please eSchedule an Advising Appointment with a Pre-Admitted Academic Advisor.
Declaration of Minor
What is it? This allows students to declare a minor in the College of Education. Students earning
a minor from the College of Education are not required to earn a major in the College
When do I use it? If you are interested in declaring a minor within the College of Education, please eSchedule an Advising Appointment with an academic advisor.
What is it? Sometimes course times overlap and OASIS will not let you register for courses with
time conflicts. This Time Conflict form needs to be signed by the professors of both classes and also requires a departmental
When do I use it? If a student would like to take classes that overlap and has received permission from the departments of both classes.
