Vaccine Option for USF Students (4/12/21)

Dear students:

We are excited to announce the University of South Florida is preparing to administer the COVID-19 vaccine to eligible USF students. The COVID-19 vaccine is not required by USF, but strongly encouraged. Vaccines can be particularly helpful if you have plans to travel in the coming weeks or months, according to CDC guidelines.

We are circulating this survey to determine the number of students interested in receiving the vaccine and to assist in scheduling. You will need to complete this survey to be contacted for an appointment. 

While our priority remains vaccinating students, staff and faculty, this survey will ask if any of your immediate family members need the vaccine. This information will be used in the event we are able to assist in vaccinating other members of our community.
